OUR DEVOTED PROFESSIONALS

 

At Caremark Healthcare, our team of professionals work collaboratively to provide very dynamic interdisciplinary team (IDT) approach. Setting standards of patient care where goals no longer focus on curative but rather palliation of patient discomforts is paramount.

Our team meets twice a month and as needed, and is led by our devoted Hospice Medical Director who oversees and approves the patient’s plan of care. In collaboration, the attending or Primary Care Physician and the Director of Patient Care Services are the key members ensuring that each of our patient’s plan of care is initiated and carried out accordingly by all the assigned field staff.

Equally instrumental to the ongoing success of Caremark Healthcare are the wonderful and friendly staff in the office and all the field clinicians as follows:

Nurses

Our Registered Nurse Case Manager is the primary liaison between the patient, the doctor(s), the family, and/or the facility staff. As an alternate skilled nurse, our Licensed Vocational Nurses (LVNs) are readily available as clinical field support for dispatch after-hours and weekends, routine visits, or anywhere as needed.

Chaplain

Our non-denominational Chaplain is the person responsible for planning and providing spiritual support to all the patients and provide bereavement services to the next-of-kin and other family members as requested.

MSW

As the psychosocial experts of the agency, our Medical Social Worker (MSW) is responsible for the overall psychosocial support and community resource liaison for the patient and the family.

Hospice Aide

Our Hospice Aide and/or Homemaker specializes in helping with the patient’s personal care needs as well as activities of daily living (ADLs)

Volunteer

A volunteer fills a special role at Caremark Healthcare. S/he may provide companionship, respite care, emotional support and therefore, promote the patient’s better sense of well-being. The ancillary staff consists of Dietary Consultants, Speech and Language Pathologist, Respiratory, Physical and Occupational Therapists.
